Nurses are classified as an “extra-quota” category under Italy's Decreto Flussi 2026–2028, alongside EU Blue Card holders, executives, university professors and translators — meaning an employer can request your work authorisation (nulla osta) at any time of year, without waiting for a capped annual quota window or competing on a click day.
This guide covers the legal basis for that exemption, the exact process from job offer to residence permit, and where people typically go wrong.
Why Nurses Don't Compete in Italy's “Click Day” System?
Italy's Decreto Flussi sets an annual cap on non-EU work visas — 164,850 for 2026 alone — and most categories compete for that capped number through a first-come, first-served online submission window known as click day. Nurses are explicitly carved out of this cap as an “extra-quota” professional category.

The Visa Process, Step by Step
Figure 1: The nurse-specific, quota-exempt visa process, from job offer to residence permit.
Because nurses aren't waiting for a quota window, the practical bottleneck shifts to documentation accuracy — the Sportello Unico per l'Immigrazione (SUI) and the Labour Inspectorate can verify the seriousness of the job offer, the employer's financial capacity, and consistency between the role, salary, contract type and workplace before granting the nulla osta.
What happens at the Sportello Unico per l'Immigrazione (SUI)?
The SUI is the provincial office that processes the nulla osta request, checking that the employer, role, and contract are genuine and consistent with each other. Once cleared, the employer has 15 days (extended from the previous 7-day window) to confirm the nulla osta after being notified that entry-visa background checks are complete — missing that window is a common, entirely avoidable cause of delay.
After You Arrive: The Residence Permit (Permesso di Soggiorno)
Entering Italy on your visa isn't the finish line. The standard rule requires you to present yourself at the Sportello Unico within 8 working days of arrival to begin your permesso di soggiorno application; under the current decree, the deadline for signing the “contratto di soggiorno” with your employer has been extended to 15 days. The visa gets you into the country; the permesso di soggiorno is what actually authorises your ongoing stay and work beyond the visa's entry window.

How Long Your Residence Permit Lasts — and Renewing It?
|
Contract Type |
Initial Permit Duration |
|
Fixed-term (tempo determinato) |
Up to 1 year |
|
Permanent (tempo indeterminato) |
Up to 2 years |
Renewal must be requested within 60 days of expiry, at your local Questura. On renewal, permits tied to a permanent contract or family reunification can be extended up to a maximum of 3 years — longer than the original grant. Missing the 60-day renewal window is a serious risk: you can lose the right to work and access to the national health service until the situation is resolved.
Family Members
Dependants generally apply for family reunification separately, once your own residence permit is confirmed — it isn't bundled automatically into your own work visa application, so it's worth planning as its own timeline rather than assuming it happens in parallel.
Common Pitfalls
- Treating this as a simple online submission — by the time you're ready to submit, your documentation should already be checked, ordered and complete, not assembled after the fact.
- Missing the employer's 15-day confirmation window after visa background checks clear.
- Missing the 15-day window after arrival to sign the contratto di soggiorno and apply for the permesso di soggiorno.
- Document mismatches — the single most common cause of rejection is inconsistency between what's submitted and what the employer's sworn declaration (asseverazione) states.
- Missing the 60-day renewal window before your permesso di soggiorno expires — this risks losing your right to work and access to healthcare until it's resolved.
